PHZQINC, LLC AUTO PAY LEGAL
By enrolling in this payment option, you are authorizing PHZQINC, LLC each month to charge your credit card, debit card, on your payment date for the total balance due on your account. The payment date and amount are indicated on your PHZQINC, LLC recurring subscription receipt. Through the subscription method you have chosen, you will also be notified of the payment date and amount due payment.
Your authorization applies to the credit card, debit card, on file, and any future information provided by your bank, or by you through, or by your contacting PHZQINC, LLC support at physiqueincteam@gmail.com. PHZQINC, LLC does not charge for this service at this time; however, you should check with your bank for any additional charges. PHZQINC, LLC reserves the right, upon notification, to terminate this program at any time. All charges, terms, and conditions are subject to change. This agreement does not alter in any way the terms and conditions of your existing Customer Agreement.
You understand that if your payment is rejected, refused, returned, disputed, or reversed by your financial institution or card issuer for any reason, then PHZQINC, LLC has the right to charge a PENALTY FEE of $200.00 and to cancel your account from the recurring payment option. If your financial institution ever declines your automatic payment for any reason then PHZQINC, LLC will contact you and retry the transaction.
You have authorized us to treat your electronic signature as evidence of your consent to initiate recurring electronic payment transactions from your credit card, debit card, or bank account on a monthly basis. This authorization via electronic signature has the same effect as a handwritten signature on a paper-formatted contract. Your consent only applies to the initiation of recurring electronic credit card, debit card, and bank account debits.
You may withdraw your consent to the monthly recurring payment transactions by contacting PHZQINC, LLC at physiqueincteam@gmail.com.
You will receive a copy of these terms electronically. In order to furnish you with a copy of your consent to monthly recurring payment transactions, we may also send a paper copy of this authorization to your home address on file with us.
From time to time, PHZQINC, LLC may revise these Terms and Conditions. PHZQINC, LLC will provide notice of such revisions by posting revisions to the website Announcements page. Material revisions to the Terms and Conditions shall be effective no sooner than 30 days after posting on the PHZQINC, LLC website at www.physiqueincteam.com/terms-and-conditions. By continuing to use the recurring payment option after revisions are in effect, you accept and agree to all revisions.

This Site is operated by PHZQINC, LLC, and materials on the Site are primarily owned by PHZQINC, LLC. Please read these Terms of Use carefully and save them. You agree that you are not entitled to any compensation, credit, or notice whatsoever and that by sending an unsolicited submission you waive the right to make any claim against PHZQINC, LLC, its parents, or affiliates relating to unsolicited submissions, including, without limitation, unfair competition, breach of implied contract or breach of confidentiality.
TERMINATION
PHZQINC, LLC reserves the right to immediately terminate your use of, or access to, this Site at any time if PHZQINC, LLC decides at its sole discretion that you have breached this Agreement or any relevant law, rule, or regulation or you have engaged in conduct that PHZQINC, LLC considers to be inappropriate or unacceptable.
COPYRIGHT INFRINGEMENT
If you believe that any material contained in this Site infringes your copyright, you should notify PHZQINC, LLC of your copyright infringement claim in accordance with the following procedure. PHZQINC, LLC will process notices of alleged infringement which it receives and will take appropriate action as required by the Digital Millennium Copyright Act (“DMCA”) and other applicable intellectual property laws. The DMCA requires that notifications of claimed copyright infringement should be sent to this Site’s Designated Agent who is: the Legal Department, PHZQINC, LLC, e-mail: physiqueincteam@gmail.com.
To be effective, the notification must be in writing and contain the following information (DMCA, 17 U.S.C. 512(c)(3)): 1.
